Team Coordinated Services
Our services are team-coordinated among our compassionate staff of professional caregivers. They include physicians, physiatrists, and psychologists. Our rehabilitation team includes occupational, speech, recreational and rehabilitation nurses. Also, an integral part of our team are case managers, dietitians and social workers. Individualized care plans and one-to-one treatment programs focus on measurable outcomes helping each patient adapt and/or recover in the shortest period of time.
A follow-up note indicating the type of services rendered is sent to your primary physician in the community upon discharge to keep them abreast of your progress.

Individualized Care Plans are established for each patient and family members are encouraged to participate.

Not Sure of the Level of Care?
Prior to admission, all cases are reviewed for the appropriate level of care by the facility case managers and the hospital case managers and the designated insurance carrier. A determination for subacute services or acute rehabilitation will be made based on severity of illness and intensity of services.

The Angela Jane Pavilion provides comprehensive subacute and acute rehabilitative services in a quiet, attractive environment. There are subacute beds, as well as acute hospital rehabilitation beds to serve patient needs.

The Angela Jane Pavilion operates according to accrediting agency criteria, using individual, case management and outcome-based quality improvement protocols.

Angela Jane Pavilion will pre-schedule your admission to coincide with your surgery date. The patient and family can tour the facility prior to admission to insure their comfort level.
As a physician, you can apply for temporary privileges if you choose to follow your patients here at Angela Jane.
